All students are required to submit the following 2 forms upon admittance to Millsaps College:

Ø       Statement of Financial Responsibility
This is a legal document obligating the student to pay any and all charges incurred during the course of their education at Millsaps College.

Ø       Student Authorization
This is a three-part authorization form regarding: I.) FERPA; II.) Federal Funds; and III.) Student Health Insurance.

I.) The Family Educational Rights and Privacy Act of 1974, 20 U.S.C. 1231g ("FERPA"), assures the confidentiality of each student's educational record.  Please authorize us to communicate with the appropriate person(s) in your family or any individual or company that will be responsible for payment of your student account balance. For additional information on FERPA refer to page 14 of the Millsaps College Catalog or http://www.millsaps.edu/records/catalog/2007-2008.pdf.

II.) The Federal Funds authorization is required when federal funds are awarded to a student account for the current academic year. This authorization must be on hand to instruct us how to process these excess funds.

NOTE: In order to use a Federal credit to obtain a bookstore voucher to purchase your books, you must authorize us to charge miscellaneous expenses against the credit balance on the authorization form.

III.) Adequate health insurance is required for all full-time students. Millsaps College is pleased to offer the Accident & Sickness Student Insurance Plan to all students. Students who can demonstrate adequate outside coverage may complete the waiver section of this form to exculde them from this plan. If you do not submit proof of coverage, you will automatically be enrolled and the insurance premium will be charged to your student account.

NOTE: This policy does not cover students while participating in SCAC/College sponsored sports. Additional coverage is required. (This restriction does not apply to intramural sports.)

Required Parent Form

Ø       F-PLUS: Federal Parent Loan
This authorization is to be submitted by any individual applying for a Federal Parent Loan which will be used to cover the student's cost of attendance. This form covers the same federal requirements as discussed in Student Authorization (ii.) but is to be completed by the borrowing parent.

NOTE: If this loan creates a credit balance on the student account and a refund is requested, the refund check is always issued in the student's name and sent to the student's permanent/home address.

All forms will be held on file in the Business Office and referenced as needed. These forms will remain in effect until a written statement revising or rescinding the listed information is submitted to the Business Office. Each spring, during pre-registration for the upcoming fall semester, students will be provided a copy of the information currently on file and may use this opportunity to make needed revisions.
